Collaborator Sample Information

This was a multiple-page form for sample submissions to the Joint Genome Institute. The JGI needs to know all kinds of important things about an oganism before it can accept DNA samples from it for sequencing. We needed a multipage form that gave the users the ability to start on one day and finish on another. Besides the interaction design, I took care of the graphics and the UI coding.
